Triphora trianthophoros (Swartz) Rydberg — Three Birds Orchid

Habitat:

Humid forests and swamps, rhododendron thickets, especially on rotten logs or on humus, in the mountains often under hemlock. The species is widespread (but scattered) in e. North America, and south into Central America.
